[Adapt] blackhole gpu is down

Dear Adapters,

We found that our GPU on blackhole is down since last week.We've tried
several times to re-install the Nvidia driver but failed.

We need more information to fix the gpu. So I'm writing this email to ask
if some of you did any operations on the blackhole's gpu last week.
